We start planning for your move to home or the next step in your recovery journey as soon as you are referred to our hospital. Our coordinated care team works with you and your family to create a brain injury or stroke recovery treatment plan specifically for you.

Conditions

We may be able to help if you have experienced any of the following:

  • Ischemic stroke
  • Intracranial Bleeding (ICB—traumatic or atraumatic)
  • Subarachnoid hemorrhage
  • Aneurysm, vascular malformations (post clipping, ligation or embolization), cerebral vasospasm and hydrocephalus
  • Traumatic brain injuries such as falls, vehicular crashes, spinal cord injury and sport-related accidents
  • Complications associated with stroke/CVA

Care Planning

Our coordinated care team develops a plan specifically for your loved one’s needs. This may include:

  • Brain monitoring (on-site EEG and CT scanning)
  • On-site surgical removal of hardware
  • Patient and family education on brain injury, stages of recovery and the patient’s specific care plan
  • Special precautions to prevent deep vein thrombosis, stress ulcers, aspiration, infection, wounds and falls
  • An environment supporting neuro recovery
  • Respiratory therapy including ventilator weaning and tracheostomy management
  • Cardiac monitoring, including telemetry
  • Nutritional assessment and support
  • Pain management
  • Rehabilitation – physical, occupational and speech therapies
  • Bowel and bladder training
  • Financial planning for extended care
